Request for Clinical Records

At the Abano Terme Hospital the patient may request a copy of their medical records for a fee. The service allows for copies of:

•    Diagnostic reports
•    X-rays
•    Emergency Room documentation
•    Medical records


Upon request, the documentation can be sent directly to the patient's home address.

 

Since this information is strictly personal and confidential, it may be delivered only to:

•    the patients themselves (when over the age of 18 years)
•    people holding a written authorization signed by the owner authorizing them to accept on their behalf and showing proper identification for both the owner (photocopy) and themselves
•    legitimate heirs upon presentation of proper auto-declaration on a special form
•    guardians of minors upon presentation of proper auto-declaration on a special form
•    legal guardians of people who are incapable of taking care of themselves upon presentation of an appropriate auto-declaration on a special form
•    INAIL (Italian Social Security Institution) for one of its insured; or the survivors of the patient according to the provisions of Article 9 of the Decree Law of the President Of Italian Republic No. 1124 of 30.6.1965

 

In every other case the medical director will evaluate the requirements foreseen by the law before releasing sensitive data.

To obtain medical records it is absolutely necessary to show identification of all of those involved. Documents will not be given to anyone who cannot provide a written consent and identification.

These rules are in place to protect the confidential information and the privacy of those involved.

If additional documentation is needed, the hospital admissions reception desk or the Medical Management Department can be contacted.
